An employee was spotting a co-worker who was using an extended boom of a powered industrial truck to unload steel beams onto the forks of the truck. A beam on the forklift got caught on 4x4 cribbing. When the employee saw the beam caught on the cribbing, he told the operator to lift the forks. The beams then slid toward the front of the forks and crushed three toes on his right foot between beams. The employee suffered three toe fractures, lacerations, and skin loss.
