Final narrative
An employee was assisting a forklift operator to load an 8-piece bundle of metal plates onto a flatbed trailer. The employee was adjusting the board on the trailer to lay the plate down while the load was above on the forks. The bundle slid/adjusted while on the forks and pinched the employee's right middle finger against a wood block, amputating the fingertip.
