On May 19, 2022, an employee was performing routine maintenance on a piece of equipment that was energized. The machine's guard had been removed to perform this task. As he reached to lubricate a chain-driven sprocket, his left thumb got caught between the sprocket and the chain resulting in amputation of the thumb.
