Employees were using a pulley rig system to install fiberglass insulation in an air fabric dome. An employee was standing on the plywood of the rig when screws fastened to it gave out, causing the employee to suffer a broken left arm, a fractured skull, and a facial laceration. Another employee suffered lacerations to the right forearm when the plywood shot up and struck them.
