An employee was using a manual pallet jack to unload a pallet of blow-in insulation from a truck at the dock. While moving the pallet jack over the lip where the dock meets the dock plate, the employee lost his footing, slipped and hit his head on the concrete ground resulting in a brain bleed. The employee was hospitalized.
