The injured employee was driving an empty forklift southbound in the warehouse tunnel. The injured employee's forklift drifted into the path of a forklift that was traveling north. The northbound forklift was pushing two empty tire racks. The left front tire of the injured employee's forklift contacted the corner of one of the tire racks then continued forward and contacted the frame and tire of the northbound forklift. This caused the injured employee's forklift to turn over onto its right side. The injured employee sustained multiple fractures to both hands and required surgical amputation of the right index and middle fingers.
