An employee was driving a maintenance cart. The cart was blocked in by a bundle of metal channels, so a forklift was used to pick up the bundle. The bundle's weight caused the forklift to tip onto its two front tires, which in turn caused the bundle to fall. The left forearm of the employee driving the maintenance cart was caught and lacerated between the bundle and the cart.
