Final narrative
An employee was operating an excavator machine to load a steel joint pipe onto a trailer. A lever was moved that caused the pipe to rotate toward the machine instead of away from it. The employee was trying to prevent the pipe from rolling into the cab when it pinned his right arm against the machine and broke it.
