On August 3, 2022, an employee was calibrating a butterfly valve with a pneumatic actuator, using an air tool mechanism to cycle the valve. (Cycling the valve means using air to power an actuator that opens and closes the valve disk.) When the employee engaged the air to cycle the valve, the closing disk caught the employee's right ring finger, causing a fingertip amputation.
