On August 3, 2022, at about 9:45 AM, an employee was standing next to the depositor machine and extended his arm while it was in the process of being flushed for a product change over. The employee's finger was caught by the depositor's extruder valve in the middle of its evolution, resulting in an amputation of the left middle fingertip at the first joint.
