On September 23, 2022, an employee was working 15 feet down in a manhole, preparing to reform the manhole with mortar. A 5-gallon bucket of mortar was being lowered down to the employee when the knot on its rope broke. It fell about 15 feet and struck the right side of the employee' s back. The employee suffered a spinal fracture in the lower back and was hospitalized.
