Final narrative
An employee was on an A-frame ladder inside a bunker, which he had entered to change a conveyor belt roller. While he was looking for a place to attach his retractable lanyard, the ladder slid forward. He fell, landing on the bunker floor and on the self-retracting lanyards attached to his body harness. He suffered broken ribs.
