Final narrative
An employee was removing a trench protective system by attaching a sling from above the trench to the panel eye hook. The hydraulic shoring system was not holding pressure, causing the employee to fall 18 feet into the trench. He sustained a back injury, a right wrist fracture, and rib fractures.
