On January 26, 2023, an employee was repairing the secondary cylinder of a forklift, which is located at the forklift's mast. The forklift's mast was jacked up so the employee could install the shims from the top of the cylinder. As the employee was installing the shims from the top of the cylinder, the jack came off from underneath the mast of the forklift, and his right hand was caught between the piston and the mast. The employee's right ring finger was amputated at the first knuckle.
