The injured employee received a forklift from a co-worker to pull the oven cart out of the oven. The forklift had two fork extenders on it. To take the cart out of the oven, the injured employee removed one fork extender because the forks do not fit under the oven cart with both extenders. The injured employee completed the task and was then placing the second fork extender back on the forklift when he dropped the extender on his fingers, crushing them between the forks and the extenders. The employee's left index and middle fingers were amputated.
