An employee was inspecting a pump inside a pipe to ensure it was working properly. Pressure was released from the pipe plug, causing the plug to be pushed forward by accumulated water. The employee's left arm became pinned between the rungs on the ladder and the pipe plug, resulting in a wrist and nose fracture as well as two tooth avulsions.
