On April 27, 2023, at approximately 10:15 PM, two employees were operating reach trucks alongside one another in a cold storage warehouse. As one employee was removing a pallet with their reach truck, the material began to tip while elevated. The injured employee exited their reach truck to move out of the way and was struck by the falling cases of material. The injured employee was hospitalized with partial paralysis due to a disc pressed against a nerve.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin.
