An employee was operating a loop injector. The loop injector had released from the diffuser at the top of the casing and started climbing up the loop toward the loop holder. While reversing the loop installer back down, the employee's glove became caught in a gear and sprocket which resulted in a left index finger amputation at the first knuckle. A finger was also fractured and his left middle finger required stitches.
