Final narrative
An employee was unloading a bulk feed delivery when the feed got stuck in the truck feed bin. The employee went to the back of the truck, activated the lid open button, and climbed the ladder to the top of the truck. He successfully knocked down the feed and was exiting the truck when the lid platform suddenly activated, causing him to lose his balance. He fell 11 feet off the truck, landed on his feet, and tumbled onto the ground. The employee suffered several broken bones in his left heel and a fracture in his right foot.
