Final narrative
An employee was operating a setting machine that places unfired clay bricks onto a kiln car. The employee was manually correcting the stacking of some bricks on the kiln car deck when he lost footing, fell off the kiln car, and landed on the floor 41 inches below. He suffered a broken right hip.
