On May 28, 2024, an employee was troubleshooting a jammed punch saw and was repositioning a sensor. The machine actuated once the sensor was repositioned. The tool/punch of the saw then struck the end of the employee's left thumb (distal phalanx) resulting in a fingertip amputation.
