Employees were changing a broken spring on the inside of a storage unit. The injured employee was taking a pipe wrench off the torsion tube when the pipe wrench slipped off the tube, causing the employee to fall backward from the second rung of a 6-foot ladder. They fell to the concrete floor and landed on their right foot, resulting in a dislocation and fracture to the right ankle.
