Employees were working to dismantle forklift ram protection columns. The columns were mounted with chemical anchors and there was adhesive in the bolt holes, so another forklift was used to perform the lift. The injured employee was working to move a column with his hand when the adhesive came loose and the columns moved. The employee's left hand was crushed between the columns. His thumb was fractured and the skin was damaged and bleeding, requiring amputation.
