A maintenance contractor employee was in the process of re-installing a newly repaired machine guard back onto a four-lane bagel-making machine by inserting a mounting bolt. He reached around the guard to secure the nut and his right fingertips were pinched against a stationary part of the machine by the pneumatic indexer. He sustained amputations to two fingertips. Lock out/tag out procedures were not used.
