On August 12, 2024 at 9:40 PM, an employee was preparing to run a job on a machine by webbing the machine when they noticed that there was a piece of film attached to the thread chain. They were manually removing it when their left thumb was pinched between the thread chain and a sprocket, resulting in an amputation to the tip of their thumb.
