An employee was monitoring a palletizer machine that stacks propane tanks when their shirt blew into the grippers on the palletizer. The employee used his right hand to brace himself and his hand was caught between the gripper and a propane tank resulting in a fracture, laceration and tendon injury to the right hand.

This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle.
