An employee was erecting formwork for a concrete wall from an aerial lift. The lift had mechanical issues and was parked. The employee climbed up from a 7-foot platform to a height of 16 feet and then anchored. The anchor failed and the employee fell 16 feet, resulting in a fractured hip, scrapes, and bruises. The employee was hospitalized.
