An employee was helping lay down insulation on a roof when he stepped on a banding system used to hold insulation in place. The banding snapped and the employee fell from the roof to a concrete slab below, striking his shoulder and the side of his head. The employee was hospitalized with fractures to his ribs and arm, as well as a head laceration.
