Final narrative
An employee was standing on the back of his truck unloading water treatment chemicals into a tank at a municipal treatment facility. As the employee was climbing on top of a 330-gallon tote to reach another tote in the rear, he lost his footing and fell backward off the truck to the concrete surface below. The employee was hospitalized for a fractured collarbone, broken ribs, a possible concussion and a brain bleed requiring surgery.
