An employee was working on the side of his tanker truck and removing four bolts. While removing the fourth bolt, the wrench slipped and he fell down onto the platform (box storage) and then onto the ground. The employee landed on his shoulder and sustained fractures to his collarbone and ribs, as well as a punctured lung.
