An employee was replacing a light fixture while on an 8-foot A-frame ladder on the second from the top rung when he was shocked and fell from the ladder. His head struck the metal work rest of a band saw before he landed on the ground. The employee sustained a head laceration, bruising, and bleeding to the back of the head. He was hospitalized for a brain bleed. The source of electricity was not locked out/tagged out at the time.
