Final narrative
An employee was operating a forklift to place two empty pallets onto a stack of empty pallets that was approximately 7.5 feet high. The employee placed the pallets on the stack, backed up, and then turned the wheel prior to lowering the forks. The forklift tipped over onto its left side. The employee's left foot became caught between the forklift and the concrete floor, resulting in hospitalization with a fractured foot.
