An employee was working as a product tree pusher. He was removing damaged product when his right hand was caught between the head sealer and the bottom seal plate on the form-fill-seal packaging machine. The employee sustained amputations to his little and ring fingers. The machine was not locked out/tagged out or guarded at the time of the incident.
