A maintenance employee was operating a center rider pallet jack with a scissor lift on the forks behind him. The employee had one hand on the steering wheel and the other on the lift. As he was driving, the pallet jack began lowering the scissor lift down unexpectedly, causing the lift to shift and catch the ring on his ring finger on a bolt on the scissor lift. The employee sustained an amputation of his left ring finger.
