Final narrative
Two employees were performing a shaft change over. The injured employee was releasing air on an air shaft valve when the second employee on the other end pulled the shaft. This action caused the shaft to go through the cardboard tube and create a pinch point between the collar of the shaft and the cardboard tube. The employee's right index finger got caught in the pinch point, resulting in a fingertip amputation.
