An employee was working to clean parts that had accumulated on a conveyor belt after the belt became unplugged. The employee plugged the conveyor belt back in, then tripped over the caster of the conveyor belt, and fell. The employee sustained a fractured hip.

She experienced numbness and sustained a gash above her left eye that required stitches. She was hospitalized with a dislocated and fractured right shoulder that required surgery, a dislocated right elbow, and injuries to her head and neck.
