Employees were moving concrete forms inside a tunnel. A form carrier is used to move the forms. While they were on a platform elevated at about 19 feet (half the total height of the tunnel), the side panel of a form with hinged sections swung in and struck them. One employee suffered a broken femur; another suffered abdominal injuries to organs and the spine.
