Final narrative
An employee was adjusting a chain on a forklift that was skewed by guardrails. There was slack in the chain and the forklift's forks were not all the way down on the ground. As weight shifted, a fork dropped, the chain tightened, and the employee's right middle, ring, and little fingers were trapped between the chain and the forklift structure. The employee was hospitalized with severe lacerations to their hand and fingers.
