Final narrative
An employee was working to lower and open the liftgate of a truck. The gate lowered, but the safety chain did not release, and the liftgate platform would not extend out. The employee went to push against the gate, when the safety chain released, causing the gate to fold outward and push the employee backward to the pavement. The employee's head struck the pavement, resulting in blunt head trauma.
