Two employees were using a forklift to reset a 1,500-pound tote of mineral powder back onto a skid. The second employee was operating the forklift while the injured employee was working to set a strap around the tote. The injured employee's little finger got pinched between the forks and a metal container, resulting in an amputation at the second knuckle.
