Final narrative
An employee was repositioning one of the dies in a 45-ton punch press for setup. The machine operator, who was assisting him, needed to move the foot pedal so he could better position a light to help. When he pushed it out of the way with his foot, the punch press actuated and closed. The press contacted the injured employee's right fingertips, resulting in amputations. The employee was hospitalized.
