An employee was lowering a cylinder assembly (approximately 8 feet long, weighing over 500 lbs.) onto a pressure tank (approximately 8 feet long, weighing over 500 lbs.) to contact threads in the pressure tank cap threads. As the employee started rotating the cylinder assembly by hand to thread onto the pressure tank cap, the unthreaded portion of the cylinder aligned and slid onto the pressure tank cap threads. The cylinder assembly dropped 1-2 inches onto the pressure tank cap, closing the gap and pinching the employee s fingers. The employee sustained amputation to three fingers.
