Power washing under the conveyor.

The Sanitation employee was attempting to power wash under the dump conveyor. The dump conveyor sits after the oven outfeed conveyor and functions to dump rejected product in this case pizza crusts off the line onto an offset conveyor to move the rejects off the line. While she was cleaning in another nearby room Maintenance techs were performing routine maintenance on conveyor of the spiral freezer which must be activated for inspection of the belt. When the Maintenance techs stopped the spiral freezer belt by pulling the cord E-stop this caused the dump conveyor to drop which is its proper functioning but in this case [REDACTED] 's body was in the line of fire of the dropping conveyor and its frame and she was caught between them. It was later discovered that [REDACTED] had failed to attach the dump conveyor 's safety chains which would have prevented it from dropping in this scenario.
