Before the incident
Employee was tightening the idler arm down on a 52 deck that was being reworked. As EE was doing this she held the puller bar (which is usually not attached at this time) up in the air because the bolt was underneath it. When she tightened the bolt the vibration caused the puller bar to fall downward striking the employee in the middle of her forehead.
