Employee was tilting a tilt stand when it came free from concrete and struck his left leg

Prior to the injury the employee was driving a forklift and struck the tilt stand causing it to break free from concrete he did not report the incident. Because of this the tilt stand was no longer in good working condition and this caused it to fail when used by the same employee who struck it with a forklift.
