Before the incident
Employee was trying to get IV access for contrast for an outpatient
What happened
IV failed on patient 's Right arm and without engaging the safety device placed the dirty needle in the wrap that came with the needle. Later needle fell out of the wrap into the towel placed in the forearm of the patient and employee didn’t notice that. While picking up the towel to switch IV on the left arm needle ended up sticking himself on right 3 rd distal finger.
