What happened

While at E1925 an account that is normally serviced by this driver they fell in between the dock of the store and their trailer. While at this account EE backs up their trailer to the loading dock uses a dock plate and unloads the trailer as normal. This account has an automatic dock plate system that is activated with the push of a button. Once the plate was in position EE got their pallet jack and began to unload the trailer. As the pallet jack was placed on the dock plate the plate collapsed inwards causing EE to fall between the dock and their trailer. Upon investigation it was seen the trailer was not fully backed up to the dock. Since the trailer was not fully backed up to the dock the dock plate did not sufficiently cover the gap between the docks and the trailer. During Supervisor visit it was noted that was less than 1 coverage on their trailer. EE normally services this account twice a week and was using their trailer they use on a daily basis.
