On [REDACTED] a team member (TM) was consolidating locations. At approximately [REDACTED] the TM noticed a pallet of radiators on the C-level rack of [REDACTED] that had shifted and fallen over. The TM decided to address the issue.

During the process the TM stepped off the [REDACTED] into the racking to adjust the pallet a technique not in line with company procedures. After adjusting the inventory the TM turned to return to the machine but stepped on cardboard and plastic covering a gap in the steel wire racking. This caused the TM 's right leg to fall through the gap in the wire mesh resulting in serious lacerations and injury.

Employee had lacerations to three parts of his right leg.
