What happened
The employee was communicating pallet needs for the day. While the pallet maker was securing the last two nails the nail gun was positioned at the very edge of the wood. This allowed the tip of the gun to push in disabling its safety mechanism. The nail missed the wood and hit the metal edge of the table ricocheting and going into the employee 's leg.
