standing part way up an extension ladder

On 6 4 24 a 6-year [REDACTED] was standing part way up an extension ladder leaned against the Drying Oven in Shell Finish when the bottom of the ladder slid out and EE road the ladder down the floor (approximately 6 ft) and then rolled off the ladder causing a right ankle injury. EE was assisted from area by another Maintenance employee and refused any first aid treatment stating they just wanted to go home. EE left the plant without annotating an injury in EPMS when clocking out. [REDACTED] EE was still in pain so took themselves to ER and was diagnosed with injuries beyond first aid.
